Parent it Forward Corporation is an independent 501(c)(3) intentionally designed to disrupt our complex healthcare system with a health literacy and life-skills program©2022, for ALL youth transitioning to adult care (HCT). This includes rare or chronic disease, underserved communities, or those with educational disabilities.
The National Committee for Quality Assurance (NCQA) Patient Centered Medical Home model (PCMH) along with the Parent Navigator Program at The Children's National Hospital, was used as a foundation the Parent it Forward program.
The program goals and objectives independently follow Centers For Disease Control (CDC)definition of Health Literacy with The Healthy People 2030 Initiative, Life Skills are categorized into three main components defined by The World Health Organization (WHO) and kept in mind the Six Core Elements of Health Care Transition 3.0(HCT).
Participants in any of the five mix and match program modules will provide a chance to open dialogue, reach whole families, improve writing and telephone skills, improve critical thinking and leave confident everyone understands what services are available and how to directly connect
